Understanding depression: more than just sadness

Depression is a common and treatable mental health condition. It affects how you think, feel and function, not just how you feel on a bad day.

Sadness or depression?

Everyone feels low sometimes. The difference is in how long it lasts, how much of life it touches and whether it lifts.

Sadness usually eases with time and support. Depression tends to stay, and it can make it hard to work, rest or connect with people, even when nothing obvious has gone wrong. It is not a weakness or a character flaw, and it is treatable.

Sadness

  • Usually linked to a specific event
  • Comes and goes
  • You can still enjoy some things
  • Eases with time and support

Depression

  • Can appear without a clear reason
  • Lasts most of the day, nearly every day
  • Interest and pleasure fade across most of life
  • Affects sleep, energy, thinking and daily functioning

What is clinical depression?

Clinical depression, also called major depressive disorder (MDD), is more than a passing low mood. Clinicians typically consider it when low mood or loss of interest lasts most of the day, nearly every day, for at least two weeks, together with other symptoms such as changes in sleep, energy, appetite or concentration, and when it affects how you function at work, at home or with people.

Depression can be mild, moderate or severe, and it can come as a single episode or return over time. Only a qualified professional can make this assessment, and you don't need a diagnosis to start counselling.

How depression affects your life

Depression rarely stays in one corner of life. It often shows up in places you might not connect with mood.

  • Work, studies and productivity

Concentration, memory and motivation drop. Deadlines slip, and simple tasks take far more effort.

  • Relationships and connection

You may pull away, feel irritable or numb, or worry that you're a burden to the people around you.

  • Physical health and energy

Fatigue, aches, headaches, and changes in sleep and appetite are common physical signs.

  • Daily routines

Getting out of bed, eating well, showering or replying to messages can start to feel impossible.

Signs and symptoms of depression

Depression looks different for everyone. These are common signs across four areas of life.

Emotional

  • Persistent sadness, emptiness or hopelessness
  • Irritability or frustration over small things
  • Feeling numb or disconnected
  • Guilt or a sense of worthlessness
  • Anxiety alongside low mood

Cognitive

  • Difficulty concentrating
  • Trouble making decisions
  • Harsh, self-critical thinking
  • Forgetfulness
  • Thoughts of death, or that life isn't worth living

Behavioural

  • Withdrawing from friends and family
  • Losing interest in hobbies
  • Neglecting responsibilities
  • Using alcohol or other substances to cope
  • Crying more often

Physical

  • Constant tiredness or low energy
  • Sleeping too much or too little
  • Appetite or weight changes
  • Unexplained aches and pains
  • Moving or speaking more slowly

If several of these have lasted two weeks or more, or they're making daily life hard, it's worth speaking to a professional. If you're having thoughts of death or self-harm, please seek help now: call Tele-MANAS on 14416 (free, 24/7) or 112.

When to seek professional help for depression

You don't need to wait until you're at your lowest. Reaching out is a sign of strength, and a professional can help you understand what might help.

  • Symptoms persist

Low mood or loss of interest has lasted two weeks or more.

  • Work or relationships are affected

Your performance is slipping or your relationships feel strained.

  • Daily life feels difficult

Getting up, eating or looking after yourself feels exhausting.

  • You're concerned about your safety

If you have thoughts of self-harm, get urgent help now: Tele-MANAS 14416 or 112.

Zenup is not an emergency service. If you are having thoughts of harming yourself or ending your life, call Tele-MANAS on 14416 or 1-800-891-4416 (free, 24/7), dial 112, or go to your nearest hospital.

Depression counselling: what to expect

Knowing what happens next makes the first step easier. Here's how counselling usually unfolds.

  1. Initial assessment

Your first session is about understanding you: your mood, sleep, energy, daily life, history and what you'd like to change. Share at your own pace.

    First session
  2. Goal-setting

Together, you define what feeling better looks like for you, whether that's more energy, better sleep, getting back to work or reconnecting with people.

  3. Therapy planning

Your therapist suggests an approach, such as CBT or mindfulness-based therapy, and how often to meet. Many people start weekly.

  4. Ongoing sessions

60-minute sessions with small, practical steps to try between them, adjusted to your energy and pace.

    Ongoing sessions
  5. Progress review

You check in on progress regularly. If something isn't working, the pace, method or therapist can change.

A gentle, realistic pace

  1. Early weeksBuilding trust, understanding your patterns and taking small first steps.
  2. Over timeMany people notice gradual changes, such as slightly better sleep, more energy or clearer thinking.
  3. Later onBuilding skills to stay well, with sessions becoming less frequent.

Recovery from depression is rarely a straight line. Good weeks and harder weeks are both part of it. Your therapist will help you keep perspective.

What you can gain from counselling

Recovery looks different for everyone, and progress is usually gradual. These are changes many people work towards.

  • Improved mood

More moments of ease and a steadier emotional balance.

  • Restored energy

Regaining the motivation to engage with life and the things you love.

  • Better sleep quality

Healthier sleep patterns and waking up more rested.

  • Stronger relationships

Reconnecting with people and communicating more openly.

  • Renewed hope

A sense of purpose and a way forward that feels possible.

  • Skills that last

Tools to notice early warning signs and stay well.

Depression therapy approaches we use

Your therapist chooses the approach, or combination, that fits your concerns and goals. Here's what each one involves.

CBT for depression

Cognitive Behavioural Therapy (CBT) is one of the most widely researched treatments for depression. It looks at how thoughts, feelings and actions keep low mood going, and gives you practical ways to break the cycle.

How it works

  1. Notice harsh or hopeless thoughts, such as β€œI always mess things up” or β€œNothing will change.”
  2. Look at the evidence and find more balanced ways of seeing things.
  3. Gradually plan small, meaningful activities to rebuild energy and motivation, a technique called behavioural activation.
  4. Build a plan for spotting early warning signs and staying well.

Therapy and medication: for moderate to severe depression, a psychiatrist may recommend medication alongside therapy. Zenup psychiatrists can assess this in a 30–45 minute online consultation.

Frequently asked questions about depression & therapy

Can't find your answer? Contact us.

What is depression counselling?

Depression counselling is professional psychological support for persistent low mood, loss of interest, hopelessness or exhaustion. Your therapist helps you understand what's behind it and build practical ways to feel better, using approaches such as CBT and mindfulness-based therapy.

How is depression different from sadness?

Sadness is usually linked to a specific event and eases with time. Depression lasts most of the day, nearly every day, for two weeks or more, and affects your sleep, energy, thinking and ability to function. It can also appear without a clear reason.

How do I know if I need help for depression?

Consider speaking with a professional if low mood or loss of interest has lasted more than two weeks, or if it's affecting your work, studies, relationships or daily routine. You don't need a diagnosis, or to feel 'bad enough', to start.

How long until therapy shows results?

It varies. Some people notice changes within a few weeks, others need longer. Depression is treatable, progress is usually gradual, and consistency matters more than speed. Your therapist will review progress with you regularly. No therapist can guarantee a result, but you won't be doing it alone.

Which therapy is best for depression?

There's no single best therapy for everyone. CBT is the most widely researched, and your therapist may combine it with mindfulness-based therapy, talk therapy or ACT depending on your needs.

Can counselling help with low motivation or grief?

Yes. Counselling gives you space to process difficult emotions, understand what's draining your motivation and rebuild strength at your own pace, including after a loss.

Do I need medication for depression?

Not always. Many people with mild to moderate depression improve with therapy alone. For moderate to severe depression, a psychiatrist may recommend medication alongside therapy. Zenup psychiatrists can assess this in a 30–45 minute online consultation.

Is online depression counselling effective?

Research suggests it is for many people. A 2022 meta-analysis of randomised trials found that psychotherapy by video was about as effective as in-person therapy for reducing depressive symptoms. Severe symptoms or a crisis may need in-person care.

What happens in my first session?

Your first session focuses on understanding you. Your therapist asks about your mood, sleep, daily life, history and what you'd like to change. You can share at your own pace and ask any questions about how therapy works.

How many sessions will I need?

There is no fixed number. It depends on your concerns, goals and progress. Many people start weekly and adjust over time, and your therapist will discuss a likely timeline after the first few sessions.

How much does depression counselling cost?

Therapy fees depend on the therapist's experience, with options for different budgets. Individual sessions last 60 minutes. Psychiatric consultations cost β‚Ή1,400 for the first visit and β‚Ή1,200 for follow-ups.

Is depression counselling confidential?

Yes. Sessions are one-on-one and never recorded, and what you share stays between you and your care team. As with any licensed mental health service, information is only disclosed if there is a serious risk to your safety or someone else's, or if the law requires it.

What if I find it hard to talk or don't know what to say?

That's very common, especially with depression. You don't need to prepare. Your therapist will guide the conversation, and you can go at whatever pace feels right. Some days you'll say a lot, other days very little, and both are fine.

How does online depression counselling work?

You book a session, then meet your therapist on a secure one-on-one video or audio call. You can join from home or any private space with a stable internet connection, from anywhere in India or abroad. Sessions are tailored to you and never recorded.

What should I do in a crisis?
